Referral for AMI-Québec/Référence pour AMI-Québec

 AMI-Québec, a community organization that provides support for families and loved ones of those suffering from mental illness, is working in collaboration with the Integrated Health and Social Services University Network for West-Central Montreal (CIUSSS) under the Division of Mental Health and Addiction Programs (DMHAP).

By completing the referral form below, a family peer support worker will communicate with the families of a loved one as soon as possible in order to:

• Offer guidance throughout a loved one’s usage of Mental Health services
• Provide information about resources available in their community

AMI-Québec, une organisation communautaire de soutien aux familles et aux proches, travaille en collaboration avec le Centre intégré universitaire de santé et de services sociaux du Centre-Ouest-de-l’île-de-Montréal (CIUSSS) sous la Direction des Programmes de Santé Mentale et Dépendance (DPSMD).

En remplissant le formulaire de référence ci-dessous, une paire aidante famille pourra communiquer avec le.s membre.s de l’entourage d’un proche dans les plus brefs délais afin de leur offrir:

• du soutien à naviguer les services de soins en santé mentale d’un proche
• de l’information au sujet des ressources communautaires pour la famille et l’entourage
